Application form REG-37 must be completed with proof of identity and $10 for each copy.
*This process can be completed by mail:
1) completed application
2) copy of proof of identity
3) check made payable to “Andover Borough” for the number of requested copies
4) include a Self-Addressed Stamped Envelope
